If you're a student in one of my classes, send your notes in here. I read every submission before deciding whether to fold it into the library, and contributors are credited where their notes are published. The form is open to readers from outside the classes too; just say so in the description.
Notes from readers who weren't in one of my classes are also welcome. Just say so in the description so I can route them through the right review track.

All fields marked with an asterisk are required. A link to the notes (Google Drive, Dropbox, OneDrive, etc.) is the easiest way to share; direct uploads can come later.
